About this role
The Transition Coordinator supports continuity of care for patients within the Adult Psychiatry and Behavioral Health (APBH) service line, aiming to reduce emergency visits and readmissions by coordinating transitions across clinical and community settings. The role serves as a central liaison between patients, internal teams, and external community resources and reports to the Office Manager of the APBH Access Center and the Director of Practice Operations.
