Transition Counselor

The Transition Counselor serves as the primary case manager for Army service members participating in the Transition Assistance Program, guiding them from initial counseling through Capstone validation. The role involves developing individualized transition plans, delivering counseling and workshops, and coordinating resources to help clients meet career readiness standards and successfully transition to civilian life.
