Transition Facilitator

The Transition Facilitator works with teens and young adults (ages 16.5 to 22) to support their move from child to adult behavioral health systems, providing guidance on employment, education, housing, finances, health, and independent living. The role involves evaluating youth, creating and facilitating service plans, collaborating with multidisciplinary partners, and handling community‑based, non‑traditional hours and documentation responsibilities.
