Transition & Follow-Up Support Worker

The Transition & Follow-Up Support Worker provides individualized, person-centered, and trauma-informed support to youth and young adults transitioning from crisis toward stability and independence. This role focuses on housing stabilization, financial empowerment, employment readiness, and ongoing follow-up support to ensure long-term success. The worker builds trust-based relationships and supports participants in navigating systems, accessing resources, and developing practical life skills.
