Transition Nurse Liaison

The Hospital-Based Transitional Nurse Liaison coordinates safe and effective transitions of care for patients within the HealthPartners enterprise hospital post-acute care network. This role serves as the primary clinical liaison among the hospital care team, patients, caregivers, and various post-acute partners to facilitate safe and efficient discharge. The position involves evaluating clinical appropriateness, coordinating referrals and resources, and ensuring the receiving team has necessary information and supplies to begin care successfully.
