Transition Specialist

The Transition Specialist provides comprehensive, culturally competent services to individuals detained in city jails, supporting their successful reentry into the community through the Pathway Home 6 program. This role involves assessment, planning, coordination, and delivery of evidence-based supports, crisis intervention, and collaboration with internal and external stakeholders to ensure continuity of care. The position focuses on maximizing participant outcomes through effective case management, wraparound services, and performance monitoring.
