About this role
Provides support to RN Case Managers and Social Workers by assisting with discharge planning, placement coordination, insurance communication, and data gathering. Acts as a liaison among patients, providers, payers, and community resources to facilitate continuity of care across the inpatient-to-post-acute continuum. Works within the hospital setting across all age groups and participates in weekend rotations and coverage.
