Transitional Care Coordinator

The Transitional Care Coordinator manages the daily hospital census of engaged patients and coordinates their care while hospitalized or in post-acute facilities. This role involves scheduling post-discharge transition of care appointments, educating patients about the WellBe program, and collaborating with hospital and skilled nursing facility personnel to ensure smooth transitions. The coordinator also supports the clinical team by retrieving medical records and providing updates on patient status.
