Transitional Housing Case Manager

The Transitional Housing Case Manager secures and places survivors of domestic violence, dating violence, human trafficking, sexual assault, and stalking into transitional housing through the TH RRH program. The role focuses on helping survivors regain empowerment and self-sufficiency by providing housing assistance, completing VISPDAT assessments, and managing partnerships with landlords in Lake County, Illinois. This position requires acting as the primary point of contact for clients while coordinating with community leaders and A Safe Place staff to ensure stable housing outcomes.
