The Transitions Coordinator - Post Acute manages clinical and benefits functions for member transitions to and from long-term care, respite nursing facility services, and HCBS waiver enrollment/disenrollment. The role partners with care coordination and waiver service teams and external stakeholders to ensure members receive appropriate supports and maintain benefit eligibility during transitions.
