Transplant Liaison - Davita IKC - Pennsylvania

The Transplant Liaison guides End-Stage Kidney Disease (ESKD) and waitlisted patients through the transplant process, advocating and coordinating support from referral to transplant completion. The role partners with transplant providers, nephrologists, dialysis teams, and community organizations to support evaluations, optimize waitlisting, and promote living donor awareness. The position is based in Reading, PA and requires regular travel within Pennsylvania two days per week with the remaining work performed from home.
