Transplant Liaison Lead - HYBRID

The Transplant Liaison Team Lead at DaVita supervises and supports the Transplant Liaison Team that guides End-Stage Kidney Disease and waitlisted patients through the transplant journey. The role acts as an advocate and coordinator between transplant centers, providers, patients, and the Integrated Kidney Care team to facilitate access to transplantation and improve program outcomes. The Lead promotes waitlisting optimization and living donor awareness while supporting teammate development and program continuity.
