Transplant Nurse Coordinator

The Transplant Nurse Coordinator for the Post-Kidney and Pancreas Transplant Service is responsible for the care coordination of 200-250 pre-kidney and pancreas transplant recipients. This role involves triaging calls, collaborating with a multi-disciplinary team, attending evaluation clinics, and providing pre- and post-transplant education and support. The position also requires reviewing labs, managing prior authorizations, and facilitating consults and consents.
