Transplant Social Worker

The Transplant Social Worker supports the Kidney/Pancreas Transplant Program by conducting comprehensive psychosocial assessments to determine patient candidacy for transplantation and presenting recommendations to the multidisciplinary Selection Committee. This role provides ongoing support and care coordination to patients on the transplant wait list, helping them and their support systems prepare for the transplant process. The position requires collaboration with healthcare professionals to develop and implement patient-centered treatment plans.
