Transportation Coordinator

The Transportation Coordinator is responsible for managing and maintaining comprehensive transportation services for PACE participants, ensuring timely and safe access to the PACE center and medically necessary appointments. This role serves as a key liaison between participants, caregivers, internal departments, and contracted transportation vendors to support uninterrupted, high-quality participant care. The position involves coordinating schedules, resolving service interruptions, and collaborating with the Interdisciplinary Team to meet participant transportation needs.
