This role involves collaborating with the Interdisciplinary Team, coordinating with transportation vendors, and utilizing scheduling software to optimize participant care and satisfaction. The position requires strong organizational and communication skills to handle scheduling changes, resolve service interruptions, and maintain accurate records in compliance with PACE regulations.
