Transportation Specialist

Midland Care Connection is seeking a Transportation Specialist to safely and reliably transport PACE participants between their homes, medical appointments, and the PACE Day Center. This role involves assisting participants with vehicle entry and exit, managing wheelchair and ambulatory transfers, and maintaining a supportive environment during transit. The specialist is also responsible for vehicle maintenance, safety checks, and communicating participant concerns or schedule changes to the care team.
