Triage and Care Navigation RN

The Triage and Care Navigation RN supports the Patient Service Center by triaging and assessing patient symptoms over the phone and assisting with the transition of care upon hospital discharge. This role connects patients with follow-up appointments and resources to improve overall health and social determinants of health. The position involves collaborating with clinical team members to ensure optimal patient outcomes and provide health education.
