Triage RN

The Triage RN is responsible for triaging patient and caregiver telephone calls, assisting with symptom palliation, and providing education and support to terminally ill patients and their families. This role involves coordinating care with physicians, case managers, and other healthcare providers while maintaining strong relationships with patients and families. The position requires managing multiple assignments, utilizing EMR systems, and demonstrating strong problem-solving and communication skills in a hospice or home health setting.
