Veteran Case Manager

The Transition House, Inc. is seeking a compassionate Veteran Case Manager to provide community-based, non-clinical suicide-prevention case management through the Veteran Connection and Suicide Prevention Program. This role involves helping Veterans reduce barriers, navigate VA and community systems, and connect with individualized services and supports to address needs such as housing instability, unemployment, and behavioral health concerns. The position requires strong interpersonal skills, experience in case management or social services, and the ability to coordinate across multiple service systems while maintaining participant safety and confidentiality.
