Veteran-Directed Care Social Work Intern

The Veteran-Directed Care Social Work Intern provides supervised experience supporting Veterans, caregivers, and authorized representatives within a self-directed, person-centered service model. The role focuses on engagement, assessment, individualized planning, care coordination, and collaboration with program partners to promote independence and dignity. Interns gain hands-on experience in case management, documentation, and advocacy while working under the guidance of a field instructor.
