Virtual Care Coordinator

Cardiac Care Alliance is seeking a full-time virtual Care Coordinator to support a growing population of medically complex patients with cardiac conditions, primarily congestive heart failure. This role involves proactive outreach, continuity of care, and patient-centered engagement through telephonic communication and technology-enabled documentation platforms. The position focuses on supporting Principal Care Management, Chronic Care Management, and Transitional Care Management services while collaborating with Registered Nurses and Cardiologists.
