About this role

This part-time, fixed-term role supports an innovative mental health program for aged care residents in Gippsland. The Wellbeing Worker delivers face-to-face and telehealth wellbeing sessions, manages referrals, and coordinates intake and assessment processes under the guidance of a Senior Practitioner.

Responsibilities

Deliver face-to-face and telehealth wellbeing sessions to aged care residents.
Support intake and assessment processes for eligible clients.
Manage referrals and provide active wait-list support strategies.
Maintain accurate client records and data collection systems.
Build partnerships with Residential Aged Care Facilities and community organizations.
