Work Life Consultant (Case Manager)

As a Work Life Consultant (Case Manager), you will partner with individuals receiving services, their families, and interdisciplinary teams to ensure each person’s goals and aspirations are honored through thoughtful, person-centered planning and support. You will serve as a key advocate, coordinator, and mentor, helping ensure services are delivered with excellence, dignity, compassion, and accountability. The role involves facilitating individualized service strategies, building strong relationships, and advocating for the dignity and independence of every person supported.
