Wraparound Facilitator

The Wraparound Facilitator manages a caseload of children, youth, and families, conducting regular meetings to assess progress toward goals and communicate updates to referring parties. This role involves collaborating with team members to develop and maintain Plans of Care and Crisis Safety Plans while ensuring adherence to the Wraparound philosophy. The position requires completing all necessary documentation in compliance with agency and County standards.
