Wraparound Facilitator

The Wraparound Facilitator develops and coordinates strength-based, family-centered teams to meet the individualized needs of children and families in accordance with the Wraparound Model. The role involves providing assessment, service planning, advocacy, and life skills training to help families achieve independence and self-sufficiency. Services are delivered with a strong emphasis on cultural competence and adherence to the Wraparound Model fidelity.
