YHDP Case Manager

The YHDP Case Manager provides intensive, individualized case management to youth and young adults experiencing or at risk of homelessness, partnering with participants to develop housing stability plans that promote long-term self-sufficiency. The role utilizes a trauma-informed, strengths-based, and youth-centered approach to help participants overcome barriers and achieve independence through stable housing, education, employment, and community connections. This position collaborates with community partners to ensure comprehensive, coordinated services while maintaining compliance with HUD, Continuum of Care, and YHDP requirements.
