Youth Tracker

The Youth Tracker is responsible for completing daily contact with assigned youth and meeting the conditions of their service plans. This role requires maintaining a flexible schedule, including evenings, weekends, and holidays, to conduct face-to-face contacts and manage youth behaviors. The position involves accurate documentation, data entry into tracking systems, and providing youth advocacy in areas such as health, education, and legal matters.
