ACT Team Member

This role provides psychosocial evaluations, diagnostic assessments, counseling therapy, and crisis intervention services within a community-based setting for individuals with severe and persistent mental illness. The position focuses on vocational planning, case management, and supportive services to enhance the individual's ability to remain in the community. The role requires strong clinical assessment skills and collaboration with an interdisciplinary team to deliver comprehensive, person-centered care.
