ACT Team Member

This direct service position requires specific knowledge of mental health and skills in crisis intervention to provide community-based services. The role involves conducting daily visits, completing comprehensive assessments, and collaborating with the ACT team to develop and implement consumer treatment plans. The position also includes providing psycho-education, acting as an advocate, and participating in rotating 24-hour crisis intervention on-call services.
