Advanced Practice Clinician

The Advanced Practice Clinician provides and coordinates primary care to SCAN members, primarily in senior living communities and skilled nursing facilities, to help keep seniors healthy and independent. The role involves conducting physical exams, managing chronic diseases, collaborating with healthcare teams, and ensuring continuity of care through direct patient interaction and virtual visits. This patient-facing position requires travel within the assigned area to deliver comprehensive medical services and support to older adults.
