The ALC Navigator role focuses on expediting discharges for patients designated as Alternate Level of Care (ALC) and ensuring teams maintain progress on discharge plans. This position provides operational oversight of ALC designation processes, collaborates with stakeholders to resolve discharge delays, and manages site-wide bed pressures. The role requires auditing systems, generating patient updates, and addressing issues at both patient-specific and systems levels.
