The Ambulatory Medication Access/Referral Coordinator provides expert-level support to the Prime Healthcare Ambulatory Services Team, managing complex ambulatory pharmacy workflows including referral processing, patient scheduling, and insurance evaluation for medication access. This role serves as a liaison between providers, pharmacies, insurance carriers, and patients to streamline medication access, reduce financial barriers, and ensure continuity of care. The coordinator independently manages patient referrals, maintains accurate records in the electronic medical record, and ensures compliance with programs like 340B.
