Jefferson Center for Mental Health
The role provides high‑level outpatient services to individuals with severe and persistent mental illnesses, delivering wrap‑around support to prevent hospitalization and promote community integration. Clinicians work on an interdisciplinary ACT team, managing caseloads, coordinating care, and connecting clients to resources while adhering to the CO‑ACT model and evidence‑based practices.
