Assessment Specialist

This role assesses and supports clients receiving home-delivered meals under the Title III Older American’s Act, conducting eligibility evaluations through phone or in-home visits. The position involves coordinating referrals for benefits and community resources, maintaining comprehensive knowledge of Home and Community Based Services, and managing client data for reporting and reimbursement. The specialist collaborates with internal staff and external agencies to ensure client needs are met while maintaining accurate documentation.
