Assessor (Part-Time)

This role assesses individual needs and coordinates services to support health, safety, and independence across an eight-county service area. The position conducts in-home and telephonic evaluations to determine eligibility for nursing home level of care, Medicaid waiver programs, and state-funded services. The assessor ensures timely documentation and provides consumers with detailed information on available programs and community resources.
