About this role
The Lead Care Manager will join Pair Team's interdisciplinary enhanced care management program to support high-need individuals (Serious Mental Illness, Substance Use Disorder, homelessness, and complex medical needs). This full-time role is primarily remote with 1-2 monthly in-person community visits and focuses on building trusting relationships to improve access to whole-person care. The position partners closely with nurses, behavioral health staff, and community specialists to support patient engagement and care continuity. Candidates should be based in Orange County, CA.
