About this role
The Lead Care Manager is a full-time role on Pair Team's interdisciplinary Enhanced Care Management program, focused on improving access to whole-person care for high-need Medicaid/Medicare populations. The role centers on building trust and engagement with individuals experiencing serious mental illness, substance use disorder, homelessness, or complex medical needs. It is primarily remote (about 90% remote) with 1-2 community-based onsite visits per month and supports Pair Team’s mission to close care gaps and improve outcomes.
