About this role
The Lead Care Manager is a full-time role on Pair Team’s Enhanced Care Management team focused on improving access and outcomes for high-need Medicaid/Medicare patients. The position supports whole-person, community-based care for individuals with serious mental illness, substance use disorder, homelessness, or complex medical needs. The role is primarily remote with occasional (1–2x/month) in-person community visits and involves close collaboration with clinical and operational teammates.
