About this role
Pair Team is hiring a full-time Bilingual Member Services Representative (Lead Case Manager) to support whole-person care for high-need Medicaid/Medicare members, including those with serious mental illness, substance use disorder, and homelessness. The role is mission-driven and part of an interdisciplinary Enhanced Care Management team focused on improving outcomes and access to community-based supports. This position is primarily remote with regular community-based in-person engagements 2-3 times per month.
