About this role
Pair Team is hiring a full-time Bilingual Member Services Representative (Lead Case Manager) to engage high-need Medicaid/Medicare members and support whole-person care for individuals with serious mental illness, substance use disorder, homelessness, or complex medical needs. The role partners with interdisciplinary clinical teams to improve access and continuity of care using a tech-enabled care model. The position is primarily remote with regular community-based on-site engagement 2-3 times per month.
