Pair Team is hiring a full-time Bilingual Member Services Representative (Lead Case Manager) to engage high-need Medicaid and Medicare patients, including individuals with serious mental illness, substance use disorder, and those experiencing homelessness. The role supports Pair Team’s mission of whole-person, community-driven care using tech-enabled tools and automation. This position is primarily remote with 2-3 in-person community visits per month and is based in California.
