Morgan's Multi-Assistance Center (Morgan's MAC) is a nonprofit collaborative care community center in San Antonio, TX, that improves the lives of individuals with disabilities and special needs of all ages through comprehensive and coordinated services. Its innovative Care Model is built on four anchors—Navigation, Community Support Services, Medical Home, and Therapy Home—and uses a family-centered approach with collaboration from MAC Members, their families, and community partners known as MACers. The center addresses both medical and non-medical drivers of health, tailoring all services to each MAC Member's specific needs through a customized navigation and tracking system called The MACNav™.
