Its innovative Care Model is built on four anchors—Navigation, Community Support Services, Medical Home, and Therapy Home—and uses a family-centered approach with collaboration from MAC Members, their families, and community partners known as MACers. The center addresses both medical and non-medical drivers of health, tailoring all services to each MAC Member's specific needs through a customized navigation and tracking system called The MACNav™.
