The Long-Term Services and Support (LTSS) Care Coordinator (Care Coach 1) at Humana Gold Plus Integrated provides field-based care coordination for Medicaid members in northern Cook County, Illinois. The role supports members and families by connecting them with appropriate resources and participating in interdisciplinary care activities for whole-person well-being. This is a patient-facing position within Humana’s community-based care services.

- Valid Driver's License
