Care Coordinator

The Care Coordinator role involves creating, planning, and organizing a team to support youth ages 0-20 with complex behavioral health and/or substance use needs through the OhioRISE Medicaid managed care program. The position facilitates child and family team meetings to help keep families together and reduce hospitalizations while ensuring youth and families have a voice in their coordinated care. This community-minded position requires connecting individuals to resources and promoting healthy people and strong communities in Southeastern and Central Ohio.
