OhioRISE Moderate Care Coordinator

The OhioRISE Moderate Care Coordinator role involves delivering community-based, wraparound care coordination for children with complex behavioral health and multi-system needs across Ohio. This position focuses on guiding families through the OhioRise program to access appropriate services, build support systems, and achieve better health outcomes through collaboration and advocacy.
