Care Coordinator

The Care Coordinator manages a caseload of Transitional Age Youth (TAY) to ensure successful engagement in services, progression through a phased program model, and maintenance of housing stability. This role focuses on coordination, accountability, and outcomes by connecting participants to specialized services and monitoring their progress toward education, employment, and permanent housing. The position serves as the primary point of contact for participants from intake through program exit, developing individualized case plans and addressing barriers to engagement.
