Care Management Coordinator

This role assists in coordinating care management services within the community to help individuals access appropriate long-term care and live at home. The position involves supporting person-centered services, monitoring consumer plans of care, and assisting care managers with assessments, service orders, and documentation. The coordinator also handles client triage, maintains records, and collaborates with an interdisciplinary team to promote and support CCC services.
