About this role

Habitat Health is seeking a Home Care Coordinator to support older adults within the Program of All-Inclusive Care for the Elderly (PACE) model. This role involves conducting in-home assessments, developing care plans, and coordinating comprehensive medical and social services to ensure safe, person-centered care. As a member of the Interdisciplinary Team, the coordinator collaborates with clinical and non-clinical staff to monitor participant needs and maintain accurate documentation.

Responsibilities

Coordinate home care services to align with participant goals and payer requirements
Monitor authorization status and maintain accurate electronic health record documentation
Perform home safety evaluations to promote participant independence and reduce risk
